E-CONSULT ONLINE ACCESS TO INFORMATION
Better access to information through the internet

Our e-consult service was primarily designed for our International patients and those who are unable to physically present to one of our offices for a comprehensive examination, diagnosis and discussion. This is an effort to facilitate the patient in learning more about options and alternatives that may be available to them. We strongly urge you to read and consider the terms and Conditions of e-Consults carefully before you register or use our service.

It should be understood that an e-consult is NOT a substitution for a clinical examination and it is NOT intended to be or represented as an online examination by itself. The opinions offered are purely based on a set of hypothetical scenarios or recollection of events and the quality of images and quality of the limited information you provide at the time of your submission. We reserve the right to alter or retract our opinions if a convincing set of contradictory information is later presented. This service is primarily designed to facilitate the prospective patient’s quest for information regarding their proposed or already rendered treatment. Our e-Consult should not be considered as the definitive or authoritative source of information on any topic.

All information associated with your e-consult submission will remain strictly confidential and will not be distributed or published unless you provide us a written consent for release of such information.
 FOR PROSPECTIVE NEW PATIENTS  OR  PROPOSED DENTAL TREATMENT


Please fill out the e-Consult Registration form and provide the 4 items required below: If you have just registered a new account, the pop-up e-consult login window may remain minimized in task bar at the bottom of your desktop screen.

1) A detailed and accurate description of the proposed treatment which includes specific teeth to be treated along with the dental terms or (please consult our glossary of terms).
2) A scanned copy of the treatment plan printout provided by your dentist is usually adequate. The American Dental Association's procedure description codes (CDT-5) are also adequate, if available to you.
3) Scanned x-ray or photograph images of the tooth/teeth. Film x-rays should be scanned in at minimum of 400dpi on a transparency capable scanner with setting set to high quality color image. Digital x-rays can be e-mailed to you from your dentist’s office and then attached and forwarded with your e-consult request. For orthodontic requests please click on Sample orthodontic records.
4) An accurate description of your health with a complete list of medications (dosage and regimen) you are currently taking or taking recently.



 FOR TREATMENT YOU HAVE ALREADY RECEIVED

1) Name and contact information of the treating dentist or specialist which has performed your dental treatment.
